Acknowledgement of Talent Release Form We ("Talent"), hereby give consent to Revollve Sports Consultancy Pte. Ltd. for the usage of our video, image, voice and interview for print and online publicity purposes. We are to receive no further compensation for the usage and I give Revollve Sports Consultancy Pte. Ltd. the rights to the ownership of the project into perpetuity. Revollve Sports Consultancy Pte. Ltd. may: 

1) Photograph, voice record, film me within the venue of the lesson conducted

2) Gain the rights of all the above-mentioned assets, make copies, and edit those in a way appropriate to film without prior notice.
